The Italian licence line is engine size and distance from shore
In Italy you can generally skipper a leisure boat without a licence if you stay within six nautical miles of the coast and the engine is no more than about 40 horsepower; go further out, or use anything more powerful, and a patente nautica is required. That is why hire boats on the Lazio coast are so often exactly 40 hp. Rome itself has no sea: most of these operators work out of Ostia, Fiumicino or ports further along the coast, and the sailing season for day trips is essentially late spring to early autumn. Check the weather the day before, as the charter will.
